Medical Record Requests

Please note that this contact form is for non-urgent requests. If you have an urgent request, please contact your physician’s office at (850) 696 4000 or send a message through our patient portal, Ontada Health.

• Medical records will no longer be available for same-day pick up, unless deemed emergent by a provider.
• Medical Records will be located at the New 9-mile location.

• For EMERGENT medical records (deemed as such by your provider), pick up will be at 9 Mile Rd, first-floor concierge desk, or Med Rec will send records by courier to our North Davis location.
• For NON-EMERGENT medical records pick up, there will be a 48-hour (2 business day) notice required for pick up at any location. Pick up will be at 9 Mile Rd, first floor concierge desk or Med Rec will send records by courier to our North Davis location.

Patients can save time and unnecessary travel by calling 850-696-4002 or completing a Release for Outside Disclosure Form to request medical records in advance for either pick up or to have them sent either electronically or mailed.

Patients should have the following information before requesting records be sent to another location.

• Provider and/or Facility Name
• Address (street, city, state, and zip code)
• Telephone Number
• Fax Number
