REFERRALS

At Woodlands Medical Specialists, we strive to make the referral process as seamless as possible for our referring providers.

Below, you’ll find the instructions for submitting referrals and a downloadable referral form for your convenience.

IMPORTANT INFORMATION FOR ALL REFERRALS:

  • Fax is the best way for Woodlands Medical Specialists to receive referrals. We use a digital fax solution creating an electronic recorded trail of any document we receive.
  • When completing the referral form: please indicate a particular physician OR first available
  • Please note: All documentation must be received prior to scheduling

Oncology:

3T MRI / PET Scan / CT Scan, Lung Cancer Screening, Echocardiogram / Ultrasound (General, Breast, Vascular), Nuclear Medicine (Including Nuclear Stress Cardiac Testing), Mammography / Breast MRI / MRI Breast Biopsy, X-Ray, DEXA Bone Density Scan.
