Woodlands is growing and our medical records department has moved. What does this mean for our patients?

• Beginning June 1st, medical records will no longer be available for same day pick up.
• Records requests will require a 48-hour (2 business days) notice for pick up at any location.Patients can save time and unnecessary travel by calling 850-696- 4002 to request medical records in advance.
• We encourage patients to request their medical records be sent electronically by calling 850-696-4002 with the following information:
  • Provider and/or Facility name
  • Address (street, city, state, and zip code)
  • Telephone number
  • Fax number
